Required Documents
APPLICATION FORMS – DS-156 & DS-157 are required for each applicant, regardless of age. All questions must be filled out with a high level of accuracy. Errors and incomplete information may result in a visa processing delay.
a) DS-156 Form –Handwritten or typed Visa Application DS-156 Forms will no longer be accepted. Visa application forms printed from the Internet and then filled out by hand or typed on a typewriter will not be accepted. Please fill out the form online (http//:evisaforms.state.gov or http//:travel.state.gov), and then print it out using a laser printer. The printout will be three pages in length and include the two-dimensional barcode page. Make sure that the barcode is printed clearly and completely, with no lines broken or torn. Only use laser jet or inkjet printers and good quality copy paper when printing the online DS-156 Form.
Please recheck the completed form to ensure its accuracy. If you must make changes, please fill out another one.
b) DS-157 Form – All applicants should also complete the Nonimmigrant Visa Supplemental Application DS-157 The form is available at http//:travel.state.gov
· Passport - (must have at least 1 year validity) bring original and a x-copy
· Photo - ONE - 2”x 2” photo - colored and printed on a white background w/out borders. Taken within the last six months. Show the applicant facing straight forward, looking neither down nor to the side, with applicant's head centered in the frame and applicant's ears exposed.
· Entry/work permit or dependent card (at least 6 months validity) bring original and a x-copy
· Letter stating your purpose to enter the United States.
· Evidence sustaining the purpose of your trip and your intention to depart from the U.S. such evidence might includes: Letter from your employer; Documents outlining your plans while in the United States
· In addition to the above requirements, you are advised to present documentation and other evidence establishing social, economic, and other ties that would compel your departure from the United States after a temporary and lawful stay.
· Students seeking F1 visas are required to submit a completed I-20 form, financial supporting documents, most recent certified school transcript and SAT or TOEFL scores. Show proof of SEVIS registration and fee paid. Please pay the $200 SEVIS fee online… www.fmjfee.com
· The above requirements are not inclusive; some applicants will be required by the consular officer to submit additional documents.
Non-immigrant visa applications are accepted ONLY on Tuesdays from 8:30 am to 10:00 am. Application fee is $131.00 dollars. The non-refundable MRV fee is paid regardless of whether a nonimmigrant visa (NIV) application is approved, refused, or abandoned. The only exception to the general rule of non-refundability is to correct post error. U. S. Embassy Koror accepts cash only. Please pay with exact change.
